Primary responsibility is to provide care coordination and case management services at a health and wellness center, promoting holistic healing. The Licensed Clinical Social Worker (LCSW) conducts assessments, provides therapy (individual, group, family), and develops personalized care plans, including crisis safety and culturally sensitive approaches.
Essential duties include maintaining a caseload with weekly face-to-face contact, assembling Coordinated Services Teams, coordinating services, providing crisis support, and collaborating with healthcare providers, community resources, and families. The role involves documentation, advocacy, community outreach, treatment modification, and supervision of staff.
Qualifications include a Master’s in Social Work, current LCSW licensure in Wisconsin, at least two years of outpatient mental health experience, and familiarity with Native American communities. Skills required are strong communication, computer proficiency, confidentiality, and a valid driver’s license.
