How you’ll contribute
- The Drug and Alcohol Outreach Counselor is responsible for planning activity and initiating contacts in the community (approximately 60 mile radius surrounding the client hospital) to enhance the visibility of the service and hospital. The Service Coordinator works collaboratively with hospital administrators, providers and nursing leadership to create awareness in the community and coordinate a continuum of care for the patients with substance use disorders.
- Provide peer support to individuals with substance use and/or substance use disorders.
- Supports, and acts as a resource, for the providers and the staff that are caring for the patient as appropriate.
- Participates with community groups focused on substance use mitigation, counseling and referrals.
- Collaborates with the community focused on interventions for this population of patients, including attending networking activities.
- Assists with development and ongoing review of policies and protocols utilizing current evidence based treatment processes.
- Provides direct assessment and collects intake information in order to assist clients seeking services.
- Maintains complete records, confidentiality and tracking system.
- Adept with insurance authorizations and pre-authorization to services.
- Management of self and time in an appropriate manner.
- Communicates effectively and professionally while balencing advocacy.
- Acts as a liaison, as needed, between physicians, staff and the hospital supervisors.
- Demonstrates knowledge and skills necessary apporpriate to the population and specific needs of the patient.
What we’re looking for
High School Diploma or GED required
CAC required (Certified Addiction Counselor)
Self-identify as an addiction survivor well-grounded in recovery for at least (2) years
Experience in a healthcare setting, or clinic, required
