LSSI is hiring a full-time Intake Coordinator at Next Steps Day Treatment, Monday through Friday. Responsible for facilitating admissions into Residential Rehab by completing assessments, completing treatment plans, treatment plan reviews, and other admission or concurrent documentation.

- Complete Level of Care Assessment (LOC) and send to LCSW/LPC/LPHA no later than a day of admission into Assessment Program.
- Obtain final signature on LOC within 72 hours of admission into Assessment Program.
- Close Assessment Program and open client into Residential Program in EHR (Electronic Health Record) after completion of LOC.
- Complete Treatment Planning Assessment (TPA) within 24 hours of admission into Residential Program and send to LSW/LPC/LPHA for review and signature; obtaining final signature on TPA withing 72 hours of admission into Residential Program.
- Complete initial treatment plan within 48 hours of admission into Residential Rehab Program and send it to LSW/LPC/LPHA for review and signature; obtaining final signature on initial treatment plan within 72 hours of admission into Residential Rehab Program.
- Complete treatment plan reviews documenting client progress toward treatment goals established during their initial treatment plan. Must be launched, completed, and signed within seven (7) calendar days from the first signature from the required credential of the last approved treatment plan (review treatment plans are approved with CADC signature.
- Complete assessment of needs or required services and identify and counsel clients on most appropriate treatment options in consideration of needs, available resources and payment options; evaluate appropriateness of available treatment options to ensure proper placement. Determine need for additional services based on the assessment and facilitate linkage to care.
- Complete required paperwork related to screenings, assessments, treatment plans, continued service criteria, discharge criteria, and other required documentation accurately and timely.
- Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest and fair manner.
- High school diploma or equivalent required.
- Bachelor’s or master’s degree in a social service field preferred.
- Three (3) to five (5) years of experience in a substance use field preferred.
- Background check clearance required.
- CADC required within 18 months of hire.
- LCSW/LCPC preferred.
- Proficient in EHR (Electronic Health Record) documentation and review required.
- Familiarity with accreditation and licensure standards required.
- Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
- Excellent organization, presentation and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
- Valid driver’s license, in good standing for the state of residency required.
- Access to reliable transportation required.
- Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
