Overview
The Care Coordinator supports clients and their families through compassionate, coordinated, and client-centered services. This part-time role (up to 30 hours per week) works collaboratively with clients, families, healthcare providers, and the internal care team to ensure continuity of care, timely access to services, and accurate documentation. In this vital role, you will serve as a central point of contact, coordinating patient care, managing appointments, and supporting clients and their families to ensure seamless quality of care. Your proactive communication and attention to detail will help foster a positive patient experience while maintaining efficient office operations.
This is a part-time position of up to 30 hours per week. Work hours may vary based on client needs and may include days, evenings, nights, weekends, or PRN coverage. Some weeks may have limited or no available hours.
Responsibilities
- Act as the primary liaison between clients, medical providers, their families, care facilities, and administrative staff to facilitate smooth communication and coordination of care.
- Schedule appointments, follow-up visits, and procedures while managing calendar conflicts efficiently.
- Maintain accurate client records by updating care charts and documentation in compliance with privacy regulations.
- Conduct wellness checks and provide ongoing client support, including coordinating transportation too and from client appointments, scheduling appointments to ensure follow-up care, and participating actively as a team member supporting care team goals.
- Provide exceptional customer service by addressing patient concerns promptly and professionally.
Required Qualifications
- Bachelor’s degree in Social Work, Community Health, Psychology, Sociology, or related social service field, OR Nursing Certificate or related healthcare credential.
- Valid driver's license and auto insurance in good standing; copies required at hire and annually thereafter
- Ability to successfully complete background check, fingerprinting, and drug screening
- Valid liability insurance up-to-date and kept current.
Experience
- Prior experience working in a medical or senior care office setting is highly preferred.
- Strong background in administrative support, including scheduling and record management.
- Office experience demonstrating proficiency with administrative tools such as client databases and basic technology tools.
- Excellent organizational skills combined with the ability to multitask in a fast-paced environment.
- Clear communication skills to interact confidently with clients, providers, families, and team members.
- Ability to work effectively in a team-based care environment.
