What will you do?
We are the trusted backbone for families, guiding them through the complexities of aging. Clients and families trust us to provide personalized care that sets us apart:
- Be the Eyes and Ears – Conduct health assessments to understand medical needs, home environments, and available resources. Regularly check in, update plans as needed, and ensure nothing slips through the cracks.
- Be the Trusted Guide – Anticipate client needs, provide personalized solutions, coordinate care.
- Be the Compassionate Advocate – Ensure collaboration between healthcare providers and family members. Oversee care quality and compliance.
Who are we looking for?
- Licensed Registered Nurse (WI) with strong medical and assessment skills
- Ability to navigate the healthcare system & advocate for patients
- Organized, proactive, and able to work independently
- Exceptional communication skills
- A passion for making a difference!
