Evaluates the psychosocial needs of patients and families to develop individualized care plans. Provides supportive counseling to patients and families to address emotional and social challenges. Coordinates referrals to community resources and services to meet financial and social needs. Collaborates with the interdisciplinary team to ensure comprehensive care. Assists patients and families with obtaining financial resources and completing necessary documentation. Provides education and assistance in completing Advance Directives. Travels to patient residences to perform initial assessments and ongoing evaluations. Communicates with caregivers and makes referrals for intra-organizational services. Provides individual and group counseling to patients and families. Evaluates bereavement needs and provides appropriate support throughout the hospice process. Maintains professional documentation in the electronic medical record. Participates in interdisciplinary team meetings and other related activities. Other duties as assigned.
Schedule: PRN As Needed
Shift: Dayshift Monday- Friday 8:30am - 5pm. Orientation of Mon-Friday dayshift for first two weeks.
Location: Supporting our teams in Seminole, Orange and Osceola Counties. Hybrid position out of the Central Florida area.
- Demonstrates ability to communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements. [Required]
- Knowledge of electronic medical record. [Required]
- Ability to follow standard precautions and uses personal protective equipment when indicated. [Required]
- Ability to complete Hospice and Training, as necessary. [Required]
- Knowledge of current counseling theories and practices in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]

- 2+ experience in the social work field [Required]
- Healthcare/hospice experience [Preferred]

Pay Range:
$41,096.13 - $76,427.64
