Lawrence Street Health Care Center is seeking a compassionate, experienced, and motivated Licensed Social Worker (LBSW or LMSW) to lead our Social Services department.

This is a vital leadership role within our skilled nursing community where you will serve as an advocate, resource, and support system for residents and families navigating healthcare transitions, emotional challenges, discharge planning, and long-term care needs.

Key Responsibilities

Lead and oversee the Social Services department within the facility
Coordinate psychosocial assessments, care planning, and discharge planning processes
Serve as a primary support resource for residents and families
Advocate for residents’ emotional, social, and mental well-being
Assist residents and families with transitions, community resources, and care coordination
Collaborate closely with nursing, therapy, admissions, and interdisciplinary leadership teams
Ensure compliance with federal, state, and facility regulations and documentation standards
Support resident rights, dignity, and quality of life initiatives
Participate in care plan meetings and interdisciplinary discussions
Help foster a compassionate, resident-centered environment throughout the community

Qualifications

Current Texas LBSW or LMSW license required
Prior experience in skilled nursing, long-term care, rehabilitation, or healthcare settings required
Knowledge of Medicare, Medicaid, discharge planning, and community resources preferred
Strong communication, documentation, organization, and crisis management skills
Ability to handle sensitive situations with professionalism, compassion, and discretion
Strong leadership and relationship-building abilities
Must be flexible to assist with occasional weekend, evening, or facility event needs
