About the Role:
As a Care Coordinator, you’ll be the heartbeat of daily operations—ensuring clients receive outstanding care and caregivers feel supported. This role requires someone highly organized, proactive, and team-oriented.

Responsibilities:

• Coordinate and manage daily scheduling of services

• Resolve client and caregiver concerns promptly

• Answer calls with warmth and efficiency

• Maintain accurate care plans and records

• Collaborate with the care team for seamless service

• Participate in on-call rotation as needed

Requirements:

• 3+ years of home care industry experience

• Experience with AxisCare or similar scheduling software preferred

• Strong multitasking, organizational, and problem-solving skills

• Excellent communication and teamwork abilities

• Comfortable with technology and quick to learn new systems

• Self-starter who goes above and beyond
