About Us

At TLC Home Health & Hospice, we are committed to providing compassionate, high-quality care to patients and families throughout our communities. We are seeking a motivated and relationship-focused Community Liaison to help grow our Home Health and Hospice programs through provider outreach and community engagement.

Position Summary

The Community Liaison is responsible for developing and maintaining referral relationships with physicians, hospitals, case managers, skilled nursing facilities, assisted living facilities, and community organizations. This role serves as a bridge between referral sources, patients, families, and our clinical team while helping increase awareness and growth of TLC services.

Responsibilities

  • Build and maintain strong referral relationships throughout the community
  • Conduct weekly in-person visits to:
  • Physician offices
  • Hospitals and case management departments
  • Skilled nursing and assisted living facilities
  • Community organizations and local businesses
  • Educate referral sources on home health and hospice services, eligibility, and benefits
  • Coordinate and participate in health fairs, community events, and lunch-and-learns
  • Deliver marketing materials and promotional items to referral partners
  • Track referral activity, follow-ups, and outreach efforts
  • Collaborate with clinical and administrative teams to ensure excellent customer service
  • Maintain knowledge of Medicare guidelines and agency services
  • Assist with social media and community outreach initiatives as needed

Qualifications

  • Healthcare marketing, home health, hospice, or medical office experience preferred
  • Excellent communication and relationship-building skills
  • Strong organizational and time-management abilities
  • Self-motivated and goal-oriented
  • Comfortable with local travel and networking
  • Reliable transportation and valid driver’s license required
  • Knowledge of the local healthcare community is strongly preferred
