Our Care Managers implement optimal support and provide creative solutions that address the physical, social, and emotional well-being of our clients. You will advocate for clients, cultivate new relationships, and resolve complex care challenges with empathy and expertise. Our Care Managers,

• Provide care management services for our senior clients including: Clinical advocacy and care coordination, placement and transition management, and resource and expert referral, while demonstrating administrative and operational excellence throughout.
• Work directly with clients, families, and their care teams.
• Possess strong knowledge of seniors' health, living conditions and resources.
• Are independent, proactive and comfortable driving to client locations.
• Meet clients at home or in facilities and attend periodic in-office training and meetings. This is not a remote role.
• Build lasting relationships, solve problems, and earn client trust.
• Possess a positive, team-oriented attitude and excellent communications skills.

Qualifications & Requirements

Education & Experience

• Bachellor's Degree in Gerontology, Social Work (MSW), Behavioral Health, Counseling, Psychology, or a Registered Nurse (RN) designation. Master’s degree preferred.
• Minimum of 2 years (5 years preferred) post-degree experience in gerontology, healthcare, or psychiatric/behavioral health.
• Proven experience in dementia care, mental health issues, and working with disabled adults under age 65.
• Ability to analyze utilization problems and implement solutions that directly influence the quality of care.

Skills & Core Competencies

• Exceptional written and oral communication skills; ability to educate both clients and professionals.
• Strong time management skills and the ability to work independently in a multidisciplinary team.
• Proficiency in Google Suite (Docs, Sheets), Microsoft Office (Word, Excel, Outlook), and database management.
• Knowledge of elder abuse reporting mandates and a strict commitment to client confidentiality.

Logistics

• Availability: Monday–Friday during business hours, plus a weekend on-call rotation every 6–8 weeks.
• Travel: Valid California driver’s license, reliable personal transportation, and active auto insurance.
