Director of Program and Evaluations LMSW/LCSW - Manhattan, NY (Hybrid) - Full-Time - Monday–Friday (9–5 or 10–6) - $95,000-$110,000 Per Year

Pay Range: $95,000-$110,000 Per Year + Full Benefits

Overview:

• We are seeking a Director of Programs & Evaluation to oversee an established 822 program and a newly launching HIV Care Coordination program.
• This role focuses on program performance, operational oversight, and data-driven decision-making, while also supporting program growth through external partnerships and referrals.

Key Responsibilities:

• Program Oversight
• Oversee the 822 program and HIV Care Coordination program (combined ~$2M budget)
• Support the build-out and scaling of the Care Coordination program (startup environment)
• Ensure all programs meet contract, grant, and regulatory requirements

Performance & Data Analysis:

• Develop and track KPIs across programs
• Monitor program outcomes, utilization, and effectiveness
• Analyze trends and produce performance reports
• Translate data into clear, actionable insights for leadership
• Ensure contract fidelity and alignment with funding requirements

Leadership & Staff Support:

• Support and guide Program Directors and program staff
• Strengthen performance management and accountability practices
• Improve workflows, efficiency, and service delivery

External Partnerships & Referrals:

• Build and maintain relationships with:
• Hospitals and clinics
• FQHCs and community health centers
• Community-based organizations
• Drive referrals, program utilization, and growth

Compliance & Reporting:

• Prepare programs for audits, site visits, and funder reviews
• Maintain accurate documentation aligned with grant requirements

What We’re Looking For:

• LMSW or LCSW required
• 4+ years in HIV, behavioral health, substance use, or healthcare programs
• 2+ years in program leadership or oversight
• Strong experience in grant-funded / contract-driven environments
• Highly analytical—comfortable working with data, KPIs, and performance metrics
• Able to turn data into clear, actionable insights
• Experience overseeing multiple programs or service lines simultaneously
• Strong relationship-builder with external partners and referral sources
• Organized, operational, and outcomes-driven

Preferred:

• Advanced Excel or data analysis skills
• SIFI certification
