Position: Home Health Coordinator / Life Plan Specialist
Families First Community Center (FFCC) is growing, and we’re looking for the right person to join our team as we continue expanding services, including Behavioral Health Home (BHH).
This is a high-impact, independent role working directly with individuals and families experiencing homelessness or housing instability throughout Hancock County, primarily the Ellsworth area.
Responsibilities Include:
- Managing and building a full caseload of approximately 25–30 clients independently and confidently
- Providing Section 13 (Targeted Case Management for individuals experiencing homelessness) and Section 92 (Behavioral Health Home) services
- Coordinating care, referrals, advocacy, and system navigation
- Completing prior authorizations and maintaining MaineCare compliance
- Submitting accurate billable units through Atrezzo
- Developing and updating individualized Life Plans
- Conducting outreach in real community settings including shelters, camps, vehicles, and other locations
- Building strong relationships with community partners
- Assisting clients with transportation, appointments, and practical needs
- Maintaining high-quality documentation and professionalism
- Collaborating with and informally supporting team members
Qualifications:
- Bachelor’s degree REQUIRED (applications without a bachelor’s degree will not be considered)
- Comfortable working with vulnerable populations, including individuals and families experiencing homelessness
- Strong organizational, documentation, and communication skills
- Ability to work independently with minimal supervision
- Reliable, consistent, and responsive, especially with email communication
- Proficient in Google Workspace and electronic systems
- Valid Maine driver’s license and reliable transportation
- Strong and consistent work history required (frequent job changes will not be considered)
Work Environment:
This position includes a combination of office-based work and community outreach, with regular travel throughout our service area.
