As a Social Worker, you will play a crucial role in providing comfort, support, and peace to patients and their families during their most challenging times.Why should you apply?Join a team of dedicated professionals who strive to provide excellent careMake a meaningful impact in the lives of patients and their loved onesOpportunity to work in a compassionate and understanding environmentLearn valuable skills in medical social work and hospice careJob Description:The hospice social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.Essential Job Functions/Responsibilities:Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicate findings to the registered nurse and other members of the interdisciplinary groupCarry out social evaluations and plan interventions based on evaluation findingsMaintain clinical records on all patients referred to social workProvide information and referral services for organization patients and families/caregivers regarding practical and environmental needsProvide information to patients and families/caregivers and community agenciesServe as a liaison between patients and families/caregivers and community agenciesMaintain collaborative relationships with organization personnel to support patient careMaintain and develop contracts with public and private agencies as resources for patients and personnelParticipate in the development of the individualized plan of care and attend regularly scheduled interdisciplinary group meetingsAssist physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issuesActively participate in quality assessment performance improvement teams and activitiesOther duties as delegated by the Clinical SupervisorPosition Qualifications:A graduate of a bachelor’s program in social work accredited by the Council on Social Work Education; master's program in social work preferredMinimum of one year's experience in health care, hospice experience preferredExperience in hospice care preferredDemonstrates good verbal and written communication, and organization skillsPossesses and maintains current CPR CertificationMust be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working orderLocation: Pinal County and the Casa Grande AreaIf you are a motivated and dynamic Social Worker with a passion for providing compassionate care, apply now to join our team at Agape Hospice - Pinal County!At Agape Hospice - Pinal County, we strive to provide excellent care and we are committed to providing life changing service to our community.
