We are seeking a Client Care Coordinator who is warm, organized, detail-oriented, and passionate about caring for the elderly. This role is ideal for someone who thrives in a fast-paced environment, communicates confidently with families and caregivers, and takes pride in delivering high-quality care and service.

Responsibilities

• Build strong relationships with clients and families
• Support and oversee caregivers to ensure excellent care delivery
• Coordinate client services, care plans, and scheduling needs
• Resolve concerns with professionalism and urgency
• Partner with leadership to improve operations and client experience
• Maintain accurate documentation and attention to detail

Qualifications

• 3+ years of healthcare care coordination experience
• 2+ years of leadership or supervisory experience
• Home care experience strongly preferred
• Strong communication and problem-solving skills
• Calm under pressure, adaptable, and highly organized

Pay: $70,000.00 - $75,000.00 per year
