Job Summary
We are seeking a compassionate and dedicated Hospice Social Worker to join our team and provide vital emotional, psychological, and social support to patients and their families during challenging end-of-life journeys. In this role, you will collaborate closely with healthcare professionals to develop personalized care plans, assess patient needs, and ensure holistic comfort and dignity. Your expertise will help ease the emotional burden of patients facing terminal illness, while also supporting their loved ones through counseling, resource connection, and crisis intervention. Duties
- Conduct comprehensive patient assessments to identify social, emotional, and psychological needs related to hospice care.
- Develop and implement individualized care plans that incorporate counseling, resource coordination, and support strategies.
- Provide individual, family, and group therapy sessions utilizing techniques such as motivational interviewing, behavioral therapy, and psychotherapy.
- Assist patients and families with crisis intervention, grief counseling, and coping strategies during difficult times.
- Collaborate with interdisciplinary teams to ensure seamless integration of social work services within overall patient care plans.
- Maintain accurate medical documentation in compliance with HIPAA regulations and organizational standards.
- Collect data on patient progress, treatment outcomes, and service utilization for clinical research and quality improvement initiatives.
Requirements
- Master’s degree in Social Work (MSW) required.
- Valid licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent in your state or region.
- Proven experience in hospice care, mental health counseling, or behavioral health settings.
- Knowledge of medical terminology, medical records management, ICD coding (ICD-10/ICD-9), CPT coding preferred.
- Familiarity with EMR systems for documentation and data collection purposes.
- Strong skills in crisis management, diagnostic evaluation, case management, and working with diverse populations including children, seniors, individuals with disabilities or developmental disabilities.
- Excellent communication skills with the ability to provide compassionate counseling in high-pressure situations.
- Ability to work independently as well as part of a multidisciplinary team within inpatient, outpatient, home & community care environments such as assisted living or nursing homes. Pay: $100.00 - $150.00 per hour
