$25 - $30 per hour, depending on experience

Job Summary:

Crown Park Rehabilitation and Nursing Center, a 200-bed Rehabilitation and Nursing Facility located in Cortland, New York, is seeking a dedicated and compassionate Social Worker to join our team. As a Social Worker, you will provide essential social work services, support, and advocacy to our residents, assisting them in their adjustment to their illnesses, disabilities, and placements. Responsibilities:

  • Provide social work services, support, and advocacy to residents to assist them in their adjustment to their illnesses, disabilities, and placements

  • Maintain information on community and health resources that can be utilized during the resident's care

  • Maintain contact with families to report on changes in health condition and for discharge planning

  • Participate in the development and implementation of and adherence to the rights and responsibilities of residents

  • Participate in the process of transferring residents to appropriate levels of medical care

  • Make arrangements for obtaining needed adaptive equipment, clothing, and personal items

  • Assist residents and families in making decisions about healthcare issues

  • Monitor room changes, records, and assist through the process

  • Participate in all resident care plan conferencing to ensure attention to emotional/social needs of all residents and help define treatment goals

  • Provide individual counseling for residents and assist with difficult emotional issues and problem-solving

  • Maintain continual communication with the interdisciplinary team regarding resident medical, behavioral, and emotional needs

  • Work seamlessly with the interdisciplinary team on daily admissions and discharges

Performance Knowledge, Skills, and Requirements:

  • Working knowledge of the practices and procedures of social case work and of the social factors related to disease and disability

  • Discharge Planning and Advanced Directives (MOLST form)

  • Knowledge and understanding of MDS 3.0

  • Skilled interviewing techniques

  • Ability to organize work effectively

  • Ability to communicate effectively both orally and in writing

  • Sensitivity to the reaction of others

  • Good observation skills

  • Ability to develop care plans, complete paperwork, and regular assessments

  • Graduation from a regionally accredited or NYS registered college or university with a Bachelor's Degree or higher in Social Work, Sociology, Psychology, Human Services, or closely related field, or equivalent work experience
