Home Care Service Agency, a busy and growing home care agency in the heart of Brooklyn, is seeking a Bilingual (Spanish-speaking) Care Coordinator / Human Resources Generalist. This dual role is ideal for a highly organized and detail-oriented professional who can effectively balance client care operations and HR responsibilities in a fast-paced environment.
Position Summary
This dual-role position supports both care coordination and human resources functions. The schedule is structured as follows:
- 3 days per week: Care Coordination
- 2 days per week: Human Resources
The ideal candidate is adaptable, proactive, and capable of managing multiple priorities while maintaining professionalism, accuracy, and confidentiality.
Essential Responsibilities
Care Coordination Duties (3 Days/Week)
- Monitor HHAExchange for missed visits, verifications, and scheduling conflicts
- Schedule and coordinate care services (HHA, PCA, RN)
- Maintain accurate documentation in HHAExchange
- Conduct client service calls and follow-ups
- Assist with staffing and care coordination needs
- Communicate with insurance companies as needed
- Advocate for client well-being and ensure continuity of care
Human Resources Duties (2 Days/Week)
- Support all employee-related HR functions
- Administer employee benefits and respond to inquiries
- Assist with employee relations and disciplinary processes
- Maintain accurate personnel files
- Ensure onboarding documentation is completed prior to start dates
- Conduct onboarding and HR processing
- Process employee separations and timely deactivation
- Monitor employee compliance (medicals, in-services, certifications)
Qualifications & Skills
- Fluent in Spanish and English (required)
- Prior experience in home care, healthcare, or HR preferred
- Experience with HHAExchange strongly preferred
- Proficiency in Microsoft Office (Word, Excel, Outlook)
- Strong organizational, multitasking, and problem-solving skills
- Excellent communication and customer service abilities
- Detail-oriented with strong data entry accuracy
- Team-oriented, flexible, and self-motivated
- Ability to handle confidential information with professionalism
- Maintain organized and accurate records
- Maintain a professional, positive, and respectful work environment
- Accept feedback and implement improvements

Pay: $25.00 - $29.00 per hour
