We are compassionate, resident-centered nursing home dedicated to providing high-quality care and a supportive environment for our residents. We are seeking a dedicated and empathetic Social Worker to join our interdisciplinary team and help support residents and their families through every stage of care.

Key Responsibilities:

  • Conduct psychosocial assessments for new and existing residents.
  • Develop, implement, and update individualized care plans.
  • Provide emotional support and counseling to residents and families.
  • Assist with admissions, transfers, discharges, and care transitions.
  • Act as a liaison between residents, families, and medical staff.
  • Ensure compliance with state and federal regulations.
  • Advocate for residents’ rights and well-being.

Qualifications:

  • Bachelor's or Master’s Degree in Social Work (BSW or MSW) required.
  • Licensed Social Worker (LSW or higher) in New York preferred.
  • Prior experience in long-term care or healthcare setting strongly preferred.
  • Strong communication, organizational, and interpersonal skills.
  • Compassionate, patient, and professional demeanor.
