Hospice Intake Coordinator

At Care Plus Hospice, we are seeking a motivated and compassionate individual to work as an Intake Coordinator to service our patients in the Phoenix, AZ and surrounding areas .

Job duties consist of, but are not limited to:

• Participates in coordination of hospice services and assigning staff members based on identified needs.
• Provides accurate and timely documentation of all consents to the Admission department.
• Support quality assurance and quality improvement
• Provides other administrative assistance and support duties as directed by the Director of Patient Care Services.
• Provides appropriate follow-up to all patients, families, and facility staff
• Complies with hospice policies and procedures, laws and regulations, standards of practice, and established work rules.
• Maintains accurate tracking systems to review documentation and ensure compliance..
• Communicates with staff and outside agencies as directed.
• Facilitates communication between patients/families and other team members.

Job Type: Part-Time to Full time

Pay: $20.00-28.00 per hour

Medical specialties:

• Hospice & Palliative Medicine

Schedule:

• Monday to Friday

Experience:

• Customer service: 1 year (Required)
• Medical terminology: 1 year (Required)
• Computer skills: 1 year (Required)
• Hospice: 1 Year (required)

Job Type: Part-time

Pay: $18.00 - $23.00 per hour
