Position: Per Diem LMSW/LMHC
Location: Dutchess County Stabilization Center, Poughkeepsie, NY
Work Schedule: 3pm to 11pm Must have weekend availability
Payrate: $34.68 per hour
Job Summary:
Job Summary: The Stabilization Center is a 100% voluntary walk-in hub for people of all ages to connect – 24/7/365 – with an integrated team of clinicians, peer specialists, registered nurses, and behavioral health professionals when they’re experiencing emotional distress, acute psychiatric symptoms, substance use challenges, family issues, and other life stressors. Individuals will have immediate access to crisis counseling and behavioral health assessments, substance use counseling, peer advocacy and support, youth and family counseling, and immediate linkages to other supportive, community-based resources. The LMSW/LMHC will work as part of a multi-disciplinary team to provide direct services to individuals, including youth, and families, to include assessments, referrals, supportive counseling, crisis intervention and discharge planning.
Job Duties and Responsibilities:
- Work in collaboration with individuals, families, team members, community providers and external partners.
- Accept referrals via warm hand-offs.
- Engage in trauma-informed and person-centered care.
- Complete and document assessments, progress notes, safety plans and other documentation in a timely manner.
- Engage in crisis de-escalation techniques and therapeutic interventions to support those in crisis and divert from higher levels of care.
- Act as an advocate to help individuals access care and services.
- Demonstrate a commitment to People USA philosophy.
- Other duties as assigned.
Job Qualifications:
- Knowledge of community and community resources.
- Experience working with diverse populations and those with multiple needs.
- Strong communication skills and ability to work in a team environment
- Ability to pass required background checks.
- Current licensure of LMSW or LMHC.
- Bi-lingual preferred.
