We are seeking a Care Coordinator to support our clients and caregivers by ensuring care plans are executed at a high level. This role is ideal for someone who is organized, proactive, and enjoys building relationships with both clients and caregivers.

Key Responsibilities

Complete in home client visits to convert prospects and sign them up for services
Serve as a primary point of contact for clients and families
Conduct in-person or virtual supervisory visits
Conduct regular client check-ins and ensure satisfaction
Support caregiver onboarding to new cases
Monitor care quality and address concerns quickly
Coach caregivers on client care tactics
Partner with scheduling to ensure strong caregiver-client matches
Document all client interactions in our management tool
Participate in monthly on-call rotation'
Field work with clients across DuPage, Cook, and Will counties

What We’re Looking For

Strong communication and interpersonal skills
Organized and detail-oriented
Able to problem-solve and stay calm under pressure
Comfortable working in a fast-paced, service environment
Prior home care or healthcare experience preferred

Compensation

$22 – $27 per hour, based on experience
