The position requires working day shifts, four days a week. The duration of the assignment is 8 weeks. Candidates must have a minimum of 1 year of experience. Acute care experience and discharge planning experience are essential. A strong preference will be given to candidates with diverse case management experience. The worker must be flexible to float between various units and work on weekends, as needed. The average case load consists of 18-24 patients, which may increase on weekends. Candidates must be skilled in care coordination and able to document care delivery effectively. Experience with the EPIC charting system is required. The clinician should also possess knowledge of critical regulations and assessments relevant to social services. This role involves working closely with an interdisciplinary team to coordinate services and ensure quality care. The candidate must be able to refer patients to various resources, including rehabilitation and long-term care facilities.
