Position Summary:
The CCRT Social Worker is a vital member of NCST’s Community Crisis Response Team (CCRT), an innovative pilot program in Newark, NJ. This program builds upon NCST’s existing community-response infrastructure, utilizing a non-law enforcement, interdisciplinary crisis intervention model. Under the three-year pilot program, the CCRT will respond to behavioral health and violence-related crises affecting individuals aged 18 and older.
As the lead community responder, the CCRT Social Worker will oversee crisis response efforts, working closely with the NCST Dispatcher and Crisis and Harm Reduction Senior Director. Additionally, this role will supervise the CCRT Outreach Worker and determine which NCST team members will co-respond based on the crisis level and type. The Social Worker will provide direct assessment, de-escalation, and immediate care coordination while operating in various community settings, including streets, homes, shelters, and outreach locations.
Essential Responsibilities:
- Collaborate with the CCRT Dispatcher, Crisis and Harm Reduction Services Senior Director, and NCST outreach staff to respond to behavioral health and violence-related crises.
- Provide trauma-informed crisis response and intervention, including suicide risk and mini-mental assessments.
- Supervise and support the CCRT Outreach Worker, ensuring effective crisis intervention strategies.
- Develop clinical protocols and procedures in collaboration with NCST’s Trauma Recovery Center and Crisis and Harm Reduction Services Senior Director.
- Build and maintain strong relationships with community partners to enhance service coordination.
- Conduct behavioral health assessments and make referrals to internal NCST teams and external resources.
- Ensure proper maintenance and operation of the CCRT response van.
- Assist with participant intakes, immediate care coordination, and case management services as needed.
- Deliver harm reduction education and services to individuals in crisis.
- Identify program inefficiencies and collaborate on solutions for continuous improvement.
- Maintain accurate documentation, including progress notes in Apricot and scheduling participant follow-ups.
- Participate in interdisciplinary case conferences and team meetings to enhance crisis response coordination.
Additional Responsibilities:
- Attend relevant training to maintain professional certification and knowledge.
- Uphold NCST policies, ensuring the confidentiality of sensitive information.
- Be available for non-traditional hours based on operational and community needs.
- Assist with special projects relevant to department functions.
- Perform other duties as assigned within the scope of the role.
Required Knowledge, Skills, and Abilities:
- Ability to travel to multiple locations; a valid driver’s license is required to operate the NCST CCRT van.
- Spanish language proficiency preferred.
- Strong time management skills with the ability to set priorities and meet deadlines.
- Proficiency in Microsoft Office and ability to effectively operate computers and common software programs.
- High attention to detail with strong follow-up and follow-through capabilities.
- Ability to remain professional and composed under stressful situations.
- Excellent oral and written communication skills.
- Strong interpersonal skills, demonstrating compassion, professionalism, and cultural competence in community interactions.
- Ability to work collaboratively with diverse groups, including co-workers, external partners, and community members.
Experience, Education, and Licensure:
- 2–4 years of experience providing direct social services to high-risk populations required.
- Experience in community-based or outpatient settings preferred.
- Supervisory experience preferred.
- Bachelor of Social Work (BSW) required; Master of Social Work (MSW) preferred.
- Licensure (LSW/CSW) through the NJ Division of Consumer Affairs is required.
