You'll help clients navigate referrals, coordinate aftercare with hospitals, and ensure smooth transitions so individuals can focus on what matters most: their wellness and recovery.
What We're Looking For
- High school diploma or GED required
- Community Health Worker license preferred
- Bilingual candidates strongly encouraged to apply
- Strong communication, organization, and people skills
- Full-time position based in Tyler
