We are currently seeking qualified candidates for a full time LVN Intake Coordinator position in the Houston area.

Job Description:
Primary function is to facilitate the intake referral process, coordinate care with the interdisciplinary team and the referral source.

Qualifications:

Graduate of an accredited school of vocational nursing
Current Texas State license as a Licensed Vocational Nurse
Current Texas Drivers License
One year experience as a Licensed Vocational Nurse, home health preferred
Nursing skills as defined as generally accepted standards of practice
Good interpersonal skills
Proof of Hepatitis profile
Experience with the intake process and procedures
Experience in verifying insurance, Medicaid and Medicare
Experience with Microsoft Office (Excel, Word) and Microsoft Outlook – required
Detail oriented and highly organized
Effective written and verbal communication
Clean background and drug screen
Reliable transportation
