We are seeking a compassionate and dedicated Hospice Social Worker to join our team. The ideal candidate will have a strong background in geriatrics and hospice care, with experience in an outpatient setting.This role involves delivering a wide range of social work services to hospice patients and their families, including:Conducting initial emotional, spiritual, and psychosocial assessmentsProviding ongoing counseling and supportOffering bereavement servicesParticipating in community education, outreach, and referralsAs a key member of the Interdisciplinary Group (IDG), the Hospice Social Worker plays an integral role in assessing patient needs, developing individualized care plans, and ensuring compliance with hospice care standards.
