Medical Social Worker (MSW) – PRN (Home Health)

Nova Health First is hiring a Medical Social Worker on a PRN basis to support patients and families across the Indianapolis metro as they navigate illness, transitions of care, and community resource needs.

What you'll do

• Conduct psychosocial assessments in the home
• Connect patients and families with community resources (financial assistance, transportation, meals, LTC planning, caregiver support)
• Support advance care planning conversations and advance directive documentation
• Assess for caregiver strain, elder abuse/neglect, and mental health concerns; refer as appropriate
• Coordinate with the interdisciplinary team and referring physicians
• Document visits and care plans in WellSky

Requirements

• Master's in Social Work (MSW) from a CSWE-accredited program
• Active Indiana social work license (LSW or LCSW)
• Minimum 1 year of medical/health care social work experience (home health, hospice, hospital discharge, or SNF preferred)
• Current BLS/CPR certification
• Valid driver's license, reliable vehicle, and auto insurance
• Strong working knowledge of Indianapolis-area community and Medicare/Medicaid resources
