Position Overview:
The EAP Intake Social Worker is the first point of contact for employees and families seeking support, playing a key role in creating a positive and compassionate first impression of EAP services. This position covers the EAP phone line and intake queue, conducts psychosocial intake assessments, triages concerns, and connects clients to EAP counselors, appropriate community resources, or external providers. The Social Worker collaborates closely with EAP counselors to ensure seamless care, provides brief solution-focused support, and helps reduce barriers to accessing services. This role is ideal for a clinically motivated social worker who values meaningful client interaction, teamwork, and professional growth. Candidates with an MSW/LSW may be eligible for clinical supervision toward licensure.
Key Responsibilities:
Meet with patients and families to assess psychosocial needs and provide appropriate support.
Gather social histories to inform medical treatment and discharge planning.
Assist patients with barriers such as financial, social, or environmental challenges affecting their healthcare.
Serve as a liaison between healthcare teams and external community agencies or support programs.
Provide solution-focused assistance related to medical conditions, hospitalization, and recovery.
Collaborate effectively with multidisciplinary teams to develop comprehensive care plans.
Document assessments, interventions, and resource connections accurately in the electronic medical record.
Qualifications & Requirements:
Bachelor’s Degree in Social Work (BSW) required.
3 years of relevant healthcare or social work experience.
As of October 5th, 2025, new hires must obtain permanent LBSW licensure; incumbents prior to this date must obtain LBSW licensure within 6 months.
Incumbents with an active LSW are required to maintain their licensure.
Strong assessment, resource development, and discharge planning skills.
Excellent communication, planning, and collaboration skills within multidisciplinary teams.
Knowledge of state and county adult and child protective legal mandates.
Understanding of growth and development principles across the lifespan.
Experience tailoring services to diverse populations across geographic, cultural, and financial backgrounds.
Computer literacy and proficiency in documenting in electronic medical records.
