PRP Counselor

We are an established PRP of 10 years and recently became accredited as an OMHC. We are looking for people who understand the importance of communication and follow up.

Job Description for Psychiatric Rehabilitation Program (PRP) Counselor

1. Review your clients’ charts weekly for relevant updates.

2. Provide PRP counseling to clients at home (offsite) and at the clinic (onsite).

3. Ensure that one of your patient’s counseling sessions is onsite except there is a waiver on record

4. Address at least one goal from patient’s rehabilitation plan at each counseling session

5. Turn in PRP contact notes weekly.

6. Turn in remaining contact notes, and timesheet latest by 3rd day of subsequent month

7. Ensure complete documentation of your service

8. Attend all scheduled meetings and trainings.

9. Perform assignments delegated by the Rehab Coordinator or Program Director, which includes, but not limited to, reviewing IRP/Reviews with clients and obtaining necessary signatures.

10. Meet with the Rehab Coordinator face to face at least weekly.

Job Types: Part-time, Contract

Pay: $25.00 per hour
