Position Overview
AAA Home Health Services, Inc. is seeking an organized, detail-oriented, and compassionate Patient / Staff Care Coordinator to join our growing home health agency. This role is essential to ensuring smooth daily operations, regulatory compliance, and high-quality patient care delivery.
The ideal candidate will be experienced in home health coordination, comfortable working with EMR systems, and confident communicating with physicians’ offices, field staff, patients, and insurance providers.
Key Responsibilities
- Coordinate and schedule skilled nursing and home health aide services based on physician orders and individualized plans of care
- Communicate with physicians’ offices to obtain signed medical orders, certifications, and plans of care in a timely manner
- Submit and follow up on insurance authorizations and pre-authorizations
- Verify monthly patient insurance eligibility
- Scheduling and reviewing EVV visits for accuracy and communicate with field staff to resolve discrepancies
- Serve as primary point of contact for patients, families, and caregivers regarding scheduling and care coordination
- Monitor patient service utilization and ensure visits align with authorized services
- Maintain accurate documentation in EMR in accordance with AHCA and Medicaid guidelines
- Ensure compliance with state, federal, and agency regulatory requirements
- Support office operations and assist with credentialing or compliance-related tasks as needed
Qualifications
- Minimum 1 year experience in home health, healthcare coordination, or medical office setting (home health strongly preferred)
- Working knowledge of medical terminology and home health regulations
- Experience with EMR systems and electronic documentation
- Familiarity with EVV systems and insurance verification processes
- Strong organizational skills with the ability to manage multiple priorities
- Excellent written and verbal communication skills
- Ability to work independently while contributing to a team-oriented environment
- Bilingual (English/Spanish)
