BG Healthcare Services is seeking a passionate and driven Community Liaison to support referral development, community outreach, and client intake efforts as we continue to grow.

Key Tasks and Duties:

• Build and maintain strong relationships with Independent Living (IL), Assisted Living (AL), low-income senior housing, and PACE programs
• Develop and maintain relationships with hospital case managers, discharge planners, social workers, physician offices, rehab facilities, and skilled nursing facilities
• Develop partnerships with local community organizations and senior-focused networks
• Represent BG Healthcare Services at community events, expos, health fairs, and network meetings
• Identify, develop, and manage referral sources to consistently generate new client leads
• Track outreach efforts and follow up on all referral opportunities in a timely manner
• Collaborate with internal teams to ensure exceptional client experience from first contact onward

What We’re Looking For:

• Experience in healthcare marketing, home care, or community outreach, especially within senior services (preferred)
• Strong understanding of healthcare referral sources and care transition processes (hospital to home, rehab to home, etc.)
• Self-motivated and goal-oriented individual who can work independently in the field
• Excellent interpersonal and communication skills with a natural ability to build rapport
• Highly organized with effective time management and follow-through
• Genuine passion for making a positive impact in the senior community

Pay: $60,000.00 - $80,000.00 per year
