Job Summary

The Social Worker/Therapist, in collaboration with, physicians, staff, and community health resources, is accountable for: accurately assessing patient/family psychosocial/socioeconomic needs and coordinating the appropriate community resources at the appropriate times across the care continuum to ensure timely care, compliance with regulatory requirements/standards. The Social Worker/Therapist will manage the care of patients assigned to them simultaneously in order to help them meet their intended outcomes. Social Worker/Therapist will complete timely psychosocial assessments on each patient assigned to them. Participate in the multidisciplinary treatment plan and team meetings. Completes established competencies for the position within designated introductory period. Other related duties as assigned

Qualifications

MINIMUM EDUCATION: Graduate of an accredited School of Social Work. LMSW required

MINIMUM EXPERIENCE: 1 year in hospital social work or an equivalent practicum experience.

REQUIRED CERTIFICATIONS/LICENSURE : Possession of current Texas State Board of Social Work Examiners

PREFERRED CERTIFICATIONS/LICENSURE: LCSW

REQUIRED COURSES/ COMPLETIONS (e.g., CPR):

  • Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of hire.
  • Non-Violent Crisis Intervention obtained within 60 days of hire or transfer into position.
