Job Description

Description: The Social Worker-Palliative Care is responsible for providing psychosocial assessments, addressing psychosocial, emotional and spiritual needs for assigned adult/geriatric patients and families facing irreversible serious illness. Provides crisis intervention, emotional support, counseling, education and advocates for patients/families. Collaborates with the interdisciplinary team in development, implementation and evaluation of the treatment plan.

Typical Duties

  • Conducts psychosocial assessments for patients/families with an irreversible serious illness.
  • Participates in the development, implementation and evaluation of patient treatment/care plans.
  • Collaborates with patients/families, clinical staff and palliative care team to identify options and goals.
  • Attend scheduled palliative care IDT’s and team meetings.
  • Maintain current community resource information regarding services available for palliative care patients/ families.
  • Maintain clinical competence for adult/geriatric palliative Social Work services and exhibits professional growth and development.
  • Participate in quality improvement initiatives.
  • Maintain electronic medical records with accuracy and in compliance with JPS standards.
  • Performs other related job duties as assigned.

Qualifications

Required Education and Experience:

  • Master’s Degree in Social Work from an accredited university.

Required Licensure/Certification/Specialized Training:

  • Licensed Master Social Worker (LMSW) by the Texas State Board of Social Work Examiners.
  • Current JPS recognized Cardiopulmonary Resuscitation (CPR) Certification and must be maintained throughout employment; may be obtained within 120 days from date of hire.

Preferred Education and Experience:

  • Experience working with life-threatening conditions, chronic disease and end of life care.
  • One (1) year experience in palliative care and/or hospice care.

Preferred Licensure/Certification/Specialized Training:

  • Licensure at the Licensed Clinical Social Worker (LCSW) level or at the LMSW level pursuing
  • LCSW license.
