PT Admissions Community Liaison

West Hickory Haven is seeking the top candidate and accepting applications for a highly motivated PART TIME Admissions Community Liaison!

The purpose of this position is to assist the Admissions Director with admissions on a daily basis. The person will be working closely with families & health care providers. We are seeking a candidate who will go out into the community to market and share what West Hickory Haven is all about!

Essential Functions of the position include, but are not limited to:

• Assist the Admissions Director with day to day tasks
• Market the facility actively in the community
• Assist with financial audits
• Interact with hospital discharge planners effectively & efficiently.
• Gather & assess pre-admission data to ensure the facility can properly care for residents.
• Interview, assess and develop individualized plans to meet residents’ needs.
• Build relationships with residents/responsible party to ensure private patient accounts are collected timely.
• Notify appropriate facility departments when a resident is admitted, transferred, discharged or moves from one room to another.
• Keep accurate records of residents’ information and follow confidentiality procedures.
• Obtain required signatures on all admission forms.
• Ability to understand the facility computer system and the software used in this department.
• Work cooperatively with all facility department supervisors and administrative staff.

Wage information:

• $20.85/hour - $22.85/hour
• Including $0.85/hour MI wage pass through until 09.30.26

Requirements for this position

• Great customer service.
• High school diploma or equivalent. Associate's Degree or higher preferred.
• Strong interpersonal and time management skills
• Preferred two years of experience in long-term care / skilled nursing facility or health care.
• Successful candidates will complement existing team members.
