About Optimize Home Care

Optimize Home Care is a growing in-home care agency focused on helping seniors and recovering patients safely remain at home. We partner closely with hospitals, rehab centers, case managers, social workers and community organizations to provide compassionate reliable care.

We are currently in startup phase and looking for a motivated Community Liaison to help build our referral network and client base from the ground up.
This role begins commission-only with a clear path to base salary once consistent agency revenue is established.

Role Overview

As Community Liaison, you’ll be responsible for developing referral relationships and generating new client admissions.

This is an ideal opportunity for someone who enjoys relationship-building, has experience in healthcare outreach or sales and wants to help grow something meaningful.

Responsibilities

• Build relationships with hospitals, rehab facilities, physician offices, case managers and community partners
• Educate referral sources on Optimize Home Care services
• Generate and manage client referrals
• Represent Optimize Home Care professionally in the community
• Track outreach activity and referral progress
• Attend networking events and in-person meetings

Compensation
Startup Phase (Commission Only):

• $200 flat per client once services begin and revenue is received
• 5% of first month collected revenue per converted client referral​
• Paid once services begin and revenue is received

Growth Path:

• Transition to base + commission once agency reaches consistent monthly revenue
• Clear milestones for salary review and advancement

Ideal Candidate

• Experience in healthcare sales, home care, hospice, or medical outreach preferred
• Strong communication and relationship-building skills
• Self-motivated and comfortable working independently
• Organized and able to track outreach activity
• Entrepreneurial mindset
