At HDSW What Matters To You, Matters To Us! We are seeking a Social Care Navigator to help individuals access community resources, benefits, and services that improve health outcomes and support independence.

Position Overview:

At HDSW the Social Care Navigator (SCN) works directly with individuals to identify needs, and social support that promote stability, wellness, and independence. This role bridges the gap between clients and the complex social service systems

???? Key Responsibilities:

  • Conduct person-centered, strengths-based assessments to identify needs related to housing, food access, nutrition and transportation

  • Facilitate referrals and warm handoffs to partnering agencies

  • Ensure continuity of care and follow up on referrals

  • Track client progress and document services

  • Educate clients about available benefits and rights

  • Complete all documentation in accordance with OMH, DOH, HUD and HDSW standards.

  • Track outcomes and service utilization for reporting



Compensation details: 23.07-24.03 Hourly Wage
