POSITION SUMMARY – LMSW / LCSW Social Worker

POSITION DUTIES – Assess patient needs; Coordinate discharge planning; Provide counseling and crisis support; Connect patients to resources; Advocate for patients; Other duties as assigned.

MINIMUM REQUIRED QUALIFICATIONS – GA LMSW / LCSW; 2+ years social work experience in an acute hospital setting

PREFERRED QUALIFICATIONS – BLS; Epic experience; ED experience; Pediatric work experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Friday-Sunday, 6p-6a
