Join our dedicated team as a Home Care Community Liaison, a vital role committed to fostering strong connections between home care services and the communities we serve. In this dynamic position, you will act as a bridge, promoting awareness of our programs, building trust with community members, and ensuring that individuals receive the compassionate support they need to thrive at home. Your energetic approach and leadership skills will help expand our outreach efforts, develop impactful programs, and strengthen community relationships to improve quality of life for those in need.
Responsibilities
- Develop and implement community outreach strategies to raise awareness of home care services through public speaking engagements, social media campaigns, and local events
- Meet monthly referral quota
- Build and maintain relationships with community organizations, healthcare providers, and stakeholders to promote collaboration and service integration
- Manage volunteer programs by recruiting, training, supervising, and recognizing community volunteers who support outreach initiatives
- Lead program development efforts by designing new initiatives that address community needs related to aging, chronic illness, or disability
- Collect and analyze data on community engagement activities to measure impact and inform future planning
- Coordinate fundraising efforts by organizing events, grant applications, and donor outreach to support program sustainability
- Oversee administrative tasks including record keeping, reporting, scheduling meetings, and managing social media platforms to maximize visibility
Requirements
- Proven experience in volunteer management and supervising teams or programs
- Strong background of Medicaid waiver program is required.
- Strong public speaking skills with the ability to engage diverse audiences effectively
- Background in program development, project management, or social work with a focus on public health or community services
- Knowledge of social media management tools and marketing strategies for outreach campaigns
- Experience in data collection and analysis for program evaluation purposes
- Familiarity with addiction counseling or mental health support is a plus
- Excellent leadership qualities with the ability to motivate teams and foster collaboration
- Demonstrated administrative experience including recruiting volunteers or staff, organizing events, and managing budgets
- Ability to teach or train others effectively in community settings
