Experteer Overview In this role, you will provide professional, patient-centered care management for at-risk patients in an acute care setting. You will lead assessment, planning, and discharge coordination to ensure continuity of care and remove barriers to access. You will document interventions, ensure regulatory compliance, and advocate for patients’ needs. You will work within a Magnet-designated hospital known for high-quality care, contributing to a mission-driven, cross-functional care team.
