Position Summary
Liberty Behavioral & Community Services, Inc. is seeking a compassionate, detail‑oriented, and community‑focused Intake Coordinator & Community Liaison (Bilingual Spanish Speaking) to support our growing programs serving individuals with intellectual and developmental disabilities.
This dual‑function role is essential in welcoming new participants, guiding families through the intake process, and strengthening relationships with community partners. The Intake Coordinator & Community Liaison will gather and manage client information, support families—especially Spanish‑speaking households—through service navigation, and represent the organization within the community.
The ideal candidate is highly organized, culturally competent, and comfortable engaging directly with participants, families, and external agencies.
Key Responsibilities
Intake & Client Coordination
- Serve as the first point of contact for prospective clients, families, and community partners
- Conduct intake interviews in English and Spanish to gather required client information
- Assist Spanish‑speaking families in understanding services, eligibility, and next steps
- Process intake paperwork accurately and in accordance with company protocols
- Ensure all admission and consent forms are completed, signed, and properly filed
- Answer and screen incoming calls and emails from prospective clients in a professional and timely manner
- Collect and organize data to support eligibility and admission processes
- Maintain accurate and up‑to‑date client records
- Coordinate with internal teams to ensure smooth onboarding of new participants
Community Liaison & Family Support
- Build and maintain positive relationships with families, caregivers, and community organizations
- Support families in navigating services, resources, and agency requirements
- Communicate regularly with families to ensure continuity of care and service satisfaction
- Represent the organization professionally at community events, outreach activities, and partner meetings
- Collaborate with service providers, regional centers, and community agencies to support participant needs
- Identify community resources and help connect families to appropriate supports
Team Collaboration & Workflow Support
- Communicate effectively with team members to resolve issues and improve workflow
- Provide feedback on intake trends, community needs, and service gaps
- Uphold confidentiality, professionalism, and organizational values in all interactions
Requirements
Qualifications
- Bilingual in English and Spanish (required)
- Experience in intake coordination, patient coordination, case management, or community outreach
- Background in healthcare, social services, or community‑based programs preferred
- Strong attention to detail and organizational skills
- Excellent communication and interpersonal abilities
- Ability to handle sensitive and confidential information with discretion
- Cultural competency and ability to work with diverse populations
Physical Requirements

- Ability to interact directly with clients, families, and community members
