Position Summary
The Hospice Intake Coordinator is responsible for managing the referral and admission process for hospice services. This role serves as the first point of contact for patients, families, and referral sources, ensuring a smooth, compassionate, and efficient transition into hospice care. The ideal candidate is highly organized, detail-oriented, and committed to delivering exceptional customer service in a healthcare setting.
Key Responsibilities
- Receive, process, and coordinate incoming hospice referrals from hospitals, physicians, families, and other sources
- Verify patient eligibility, insurance coverage, and required documentation for admission
- Communicate effectively with patients, families, and interdisciplinary team members regarding services and admission status
- Coordinate with clinical staff to schedule timely patient evaluations and admissions
- Maintain accurate and up-to-date patient records in the electronic medical record (EMR) system
- Ensure compliance with all regulatory requirements, company policies, and HIPAA guidelines
- Provide compassionate support and clear information to patients and families during the intake process
- Track and report referral and admission metrics as needed
- Collaborate with marketing and clinical teams to ensure seamless care transitions
Qualifications
- High school diploma or equivalent required; associate or bachelor’s degree in healthcare administration or related field preferred
- Minimum 1–2 years of experience in healthcare intake, admissions, or customer service (hospice or home health experience preferred)
- Knowledge of Medicare, Medicaid, and insurance verification processes
- Strong organizational, multitasking, and problem-solving skills
- Excellent communication and interpersonal skills
- Proficiency in EMR systems and Microsoft Office Suite
- Ability to work in a fast-paced, emotionally sensitive environment
Preferred Skills
- Experience with hospice regulations and eligibility criteria
- Bilingual (preferred but not required)

Work Environment
This position is typically based in an office setting but may require occasional interaction with clinical teams and external partners. The role involves frequent phone and computer use.
