Job Summary:

The Nurse Care Manager (NCM) provides comprehensive care coordination and direct clinical support for uninsured and underserved patients with complex medical and social needs. This role blends population health management with hands-on nursing care, ensuring patients receive high-quality, patient-centered, and culturally appropriate services. The NCM works closely with providers, pharmacists, behavioral health staff, and community partners to remove barriers to care, promote health equity, and improve patient outcomes.

Key Responsibilities

Care Management & Coordination:

*Conduct comprehensive assessments of patients’ medical, behavioral, financial, and social needs.

*Develop individualized care plans in collaboration with patients, families, and the care team.

* Provide ongoing follow-up (in person, by phone, or virtually) to monitor adherence, progress, and outcomes.

*Facilitate transitions of care (e.g., hospital to home/clinic), ensuring medication reconciliation, continuity of services, and patient understanding.

* Identify and address barriers to care, including lack of insurance, financial hardship, health literacy, language, transportation, or access to specialty care.

* Connect patients to community resources such as prescription assistance, financial aid, food programs, and housing support.

Clinical Duties:

* Perform direct patient care within nursing scope of practice, including triage, vitals, immunizations, wound care, patient education, and chronic disease monitoring.

* Support medication management, including patient teaching, adherence counseling, and side effect monitoring.

* Provide nurse visits for chronic condition follow-up (e.g., hypertension, diabetes, asthma, depression).

* Participate in regular huddles with providers and care team to identify patient needs and priorities.

* Assist providers with procedures, testing, or urgent patient care needs.

* Maintain accurate and timely documentation in the electronic health record (EHR).

Collaboration & Quality Improvement:

* Advocate for patients who face systemic and financial barriers to care.

* Serve as a trusted liaison between patients, providers, and interdisciplinary staff.

* Participate in quality improvement projects aimed at reducing health disparities and improving outcomes for uninsured patients.

* Support health education programs and outreach initiatives to promote preventive care and self-management.

Qualifications

* Licensed Registered Nurse (RN) in Illinois

* Minimum 2 years of clinical nursing experience (primary care, community health, case

management, or care coordination strongly preferred).

* Experience working with uninsured, low-income, or culturally diverse populations strongly preferred.

* Bilingual (Spanish/English) preferred

* Strong communication, patient advocacy, and problem-solving skills.

* Familiarity with chronic disease management, social determinants of health, and

local resources.

Working Conditions

* Part-time position; combination of direct patient care and administrative/care management duties.

* Work is performed in a clinic setting, with occasional home visits or telephonic follow-up.

* Requires flexibility, teamwork, and the ability to manage multiple priorities in a dynamic, resource-limited environment.

Pay: $33.00 - $36.00 per hour
