Position Summary
The Care Manager is responsible for overseeing clinical operations, resident care coordination, regulatory compliance, and staff supervision across five Caring Arms Adult Family Homes. This role ensures high-quality, person-centered care while maintaining compliance with state regulations and company standards. The Care Manager serves as a liaison between residents, families, providers, and leadership.
Key Responsibilities
Clinical Oversight & Resident Care
Conduct comprehensive resident care plans and develop individualized care plans.
Ensure care plans are current, accurate, and implemented appropriately.
Monitor residents’ health status and coordinate with physicians, hospice, therapy, and other providers.
Review incident reports and implement corrective actions when necessary.
Support medication management processes and ensure compliance with delegation requirements.
Regulatory Compliance
Ensure all five homes remain compliant with state Adult Family Home regulations.
Prepare homes for inspections, audits, and licensing reviews.
Maintain required documentation and ensure timely completion of reports.
Implement policies and procedures consistent with state and federal standards.
Job Type: Full-time
Pay: $26.00 - $28.00 per hour
Expected hours: 40 per week
