A fully approved Michigan School Social Worker is sought for a contract role serving students in grades 6 through 8 in Grand Rapids, MI. This position involves providing direct social work services, conducting consultations, and completing evaluations within a dynamic school setting. The role requires managing a caseload of 35-40 students, with approximately 20 receiving direct services.

Key Responsibilities:

• Deliver direct social work services to middle school students.
• Consult with educators, families, and community resources to support student needs.
• Conduct comprehensive social-emotional evaluations.
• Manage Medicaid logging accurately and efficiently.
• Write Individualized Education Program (IEP) documents, including Present Levels of Academic Achievement and Functional Performance (PLAAFP), goals, and data collection.
• Maintain thorough documentation and compliance with state and federal regulations.

Qualifications:

• Full Michigan School Social Worker Approval is required.
• Experience working with middle school students (grades 6-8).
• Strong background in direct social work, consultation, and student evaluations within school settings.
• Familiarity with Medicaid billing processes and logging.
• Proficient in IEP writing and documentation.
• Ability to manage a caseload effectively and prioritize responsibilities.
