Public Partnerships LLC (PPL) helps people with disabilities, chronic illnesses, or other long-term health conditions stay at home and “self-direct” their care. Known as consumer direction in New York, this long-term care model empowers people to take control of who provides their services and where. PPL was selected to be the Statewide Fiscal Intermediary for the New York Consumer Directed Personal Assistance Program (CDPAP) starting in 2025. We, along with a diverse alliance of service partners across the state, will be supporting the delivery of culturally sensitive and disability competent care to CDPAP participants. Job Summary

The Community Liaison promotes PPL’s mission by building relationships and increasing awareness of the Consumer Directed Personal Assistance Program (CDPAP) across the NY state. This role engages with community partners and stakeholders and identifies opportunities to improve service delivery. Through outreach, education, and collaboration, the Community Liaison ensures that PPL is a trusted resource across diverse communities.


Key Responsibilities


Community Engagement & Outreach

  • Represents PPL and the state program at community outreach events and other opportunities to build awareness or support for the program.

  • Participates in various community stakeholder events and meetings that yield the opportunity for the Community Liaison to meet with program participants and stakeholders on the program and relay feedback on their experience with PPL.

  • Attends regional events as necessary to build the organization’s brand across the state.

  • Help improve or strengthen the organization’s image in the community by supporting our mission and philosophy.

  • Organizes and coordinates community events with staff.

Relationship Building & Partnership Development

  • Provides linkages and referrals from other community-focused programs and organizations to PPL.

  • Collaborates cross-functionally with other departments in PPL to facilitate even stronger service delivery and relationships with external organizations.

  • Participants in committees, education, and workforce initiatives.


Needs Assessment & Program Improvement

  • Identifies gaps in services and brings back resources for staff and or clients.

  • Identifies and creates initiatives within the organization.

  • Assesses existing needs and wants in the community and with our partners.

  • Assesses and assists in utilizing available data and how it may be used in the context of our community presence.

Training, Education & Presentations

  • Creates and delivers presentations and/or training to staff, partners, and program participants.


Required Skills and Abilities

  • Strong interpersonal, engagement, organizational, and time-management skills.

  • Commitment to supporting diverse and underserved groups of individuals.

  • Entrepreneurial approach to relationship development with external stakeholders.

  • Empathetic approach and desire to solve participant issues and challenges.

  • Capacity to display understanding and sensitivity to cultural diversity.

  • Ability to work independently with little supervision.

  • Strong written and verbal communication skills.

  • Proficient in Microsoft Office Suite.

Qualifications

  • Education: A Bachelor's degree in Social Work, Human Services, or Sociology strongly preferred. Substantial professional experience in a related field may be considered in lieu of a formal degree.

  • Experience: 4+ years of relevant experience in community engagement efforts related to delivering self-determination, job development, and support-group resources to diverse and underserved communities; bilingual in Spanish preferred

Working Condition: Remote position with regular travel requirements within the NY Western region with occasional travel to other markets.

Compensation: $70,000 - $75,000 annually
