Overview
Lincoln Glen Manor is seeking a dedicated and organized Resident Care Coordinator to manage and coordinate resident care activities across our Assisted Living, Memory Care, and Independent Living neighborhoods. This role supports high-quality, person-centered care by working with care teams, healthcare professionals, residents, and families. The ideal candidate has experience in senior care, strong communication skills, and the ability to lead a care team effectively.
Duties
- Coordinate resident care services to meet individual needs and care plans
- Participate in resident assessments and service planning
- Monitor resident health and well-being, reporting changes to nursing or wellness leadership
- Support and collaborate with nursing, caregiving, and support staff teams
- Communicate with residents and families about care needs, changes, and updates
- Assist in scheduling care staff and ensuring adequate staffing levels
- Document resident care activities in appropriate systems accurately
- Provide guidance or training to care team members on care procedures and documentation
- Assist with resident care needs as required, supporting safe and comfortable living environments
- Cover open shifts as needed to ensure proper staffing is in place
- Uphold infection control, safety, and regulatory standards
Skills and Qualifications
- Previous experience in assisted living, memory care, long-term care, or related setting preferred
- Strong communication and organization skills
- Ability to work collaboratively with multiple care disciplines
- Familiarity with care planning and care documentation systems preferred
- Understanding of resident care standards and person-centered approaches
- Ability to handle multiple priorities in a fast-paced environment
- Dependable, professional, and detail-oriented
