The Licensed Mental Health Professional will join an innovative Safe Options Support (SOS) team providing comprehensive care to street homeless or subway-dwelling individuals in the community.
The multidisciplinary team includes a leader, clinicians, care managers, and a peer specialist, utilizing evidence-based Critical Time Intervention to support participants before and after housing placement.
Key responsibilities include street outreach, coordinating needs, enhancing daily living skills, offering supportive counseling, and advocating against discrimination and healthcare inequities.
The role emphasizes member choice, harm reduction, non-coercion, flexibility, and person-centered care. The team provides ongoing support post-housing to promote stability and independence.
This field-based position requires flexible hours, periodic on-call work, and travel within three counties. It offers full-time hours, competitive pay, and benefits including health insurance, retirement plans, and paid time off. Relocation to Norwich, NY, is required before starting.
