We are seeking a qualified Bengali-speaking Case Manager to join our team at our Jackson Heights location. This role is responsible for coordinating and managing home care services for clients while ensuring compliance with regulatory, payer, and agency requirements.

Key Responsibilities

• Manage a caseload of home health members
• Conduct initial assessments and o ongoing reassessments
• Develop, implement, and maintain individualized care plans
• Serve as the primary point of contact for Bengali-speaking clients and their families
• Monitor service delivery to ensure quality, compliance, and client satisfaction
• Address and resolve care-related concerns in a timely manner
• Ensure accurate and timely case documentation
• Coordinate with medical providers, behavioral health professionals, community agencies, and social service partners

Qualifications

• Prior experience in case management or a related healthcare role
• Fluency in Bengali required
• Proficiency with RMA Foothold software required
• Strong written and verbal communication skills
• Excellent organizational and time-management abilities
• Ability to manage multiple cases in a fast-paced environment
• Must be willing to travel

Salary:

Salary commensurate with experience; range: $22–$24 per hour.

Pay: $22.00 - $24.00 per hour
