Job Title: Social Worker

Job Summary:
We are seeking a compassionate and dedicated Social Worker to support individuals and families by assessing needs, providing counseling, and connecting clients with appropriate resources. The Social Worker will advocate for clients, promote well-being, and collaborate with multidisciplinary teams to ensure comprehensive care.

Key Responsibilities:

  • Assess clients’ psychosocial needs through interviews, evaluations, and case reviews
  • Develop and implement individualized care or treatment plans
  • Provide counseling, crisis intervention, and emotional support
  • Connect clients with community resources such as housing, financial assistance, healthcare, and support services
  • Advocate for clients’ rights and access to services
  • Maintain accurate, confidential documentation and case notes
  • Collaborate with healthcare providers, educators, legal professionals, and community agencies
  • Monitor client progress and adjust plans as needed
  • Ensure compliance with ethical standards, laws, and organizational policies

Qualifications:

  • Strong communication, assessment, and problem-solving skills
  • Ability to work independently and as part of a team
  • Knowledge of community resources and social service systems

Pay: $20.00 - $23.00 per hour

Expected hours: 40.0 per week
