This is a full-time, on-site role for a Care Coordinator located in Carmel, IN. As a Care Coordinator, you will manage the scheduling and matching of caregivers to clients, provide direct support to both clients and their families, and ensure the delivery of high-quality in-home care services. Responsibilities also include maintaining communication with caregivers and clients, resolving service-related concerns, documenting client information, and ensuring compliance with company policies and procedures. You will play a key role in fostering positive relationships and delivering a caring experience to all stakeholders.
- Strong organizational, time management, and scheduling skills
- Excellent communication and interpersonal skills, with the ability to provide emotional support and build strong relationships
- Problem-solving abilities and attention to detail to address and resolve client and caregiver concerns
- Proficiency in using office software, scheduling tools, and record-keeping systems
- Experience in caregiving, healthcare, or social work is a plus
- Compassionate, patient, and empathetic approach to working with clients and families
- High school diploma or equivalent (Associate's or Bachelor's degree in a related field preferred)
